Doctor Checklist

At your appointment, your doctor may ask you a number of questions in order to properly diagnose your pain. Here are some things you may want to be prepared to discuss, as well as a list of questions you might consider asking your doctor. Print this page and take it with you to your appointment.


How are you feeling?

  • Level of discomfort over the last week
  • How you're feeling with this particular level of pain
  • The lowest and highest levels of pain you have experienced in the last day or two
  • The location of your pain, and whether or not the pain radiates (appears connected) to other areas of your body

What effect is the pain having on your daily life?

  • General activity. Your ability to do things you normally do
  • Ability to sleep. Getting to sleep, staying asleep, and how refreshed you feel in the morning
  • Enjoyment of life. The pleasure you take in your daily life
  • Normal work. Including work outside the home and around the house
  • Ability to walk. How well are you able to get around?
  • Mood. Has pain made you irritable, angry or depressed?
  • Relationships with others. How has pain affected the way you deal with other people in your life?
